Name?
Simply to call them by their name which is much more formal than their IGN.
IGN?
So you know who they are on the server and, if you have a whitelist, to whitelist them.
Contact information [Email, Discord etc.]?
An obvious one. You'll need to get in touch with your possible Admin, so having their contact information will make it significantly easier.
Location & time zone?
Just to give you an idea of where they are and what time they will generally be on the server.
What position are you aiming for?
This part is obvious. You want to know what rank the player is hoping to achieve with their application.
Age?
Age is always important. You want to know if you're hiring a 6 year old to do a 18 year old's job.
Dedication?
Dedication is vital for a good server. Staff need to be active a fair amount of time to show that they are serious about the server and they are trustworthy.
How mature are you?
Maturity is an important factor too. You want to know how mature your possible Admin is so you can get a good idea of how they will handle certain situations.
Why should I accept you?
This is probably the most important section of the application. You want to know what the player can offer to the server and you. It also gives a good view of how good their English is.
Have you been banned before, and if yes, what for?
You want to know if your possible Admin has been banned before and what for simply because you do not want a griefer having access to build in your spawn. If it was a while back, it is up to you if you give them the benefit of the doubt.
Proof of work?
This section is usually for builders or anything to do with helping a server with builds, but I always implement this in my applications as it gives the staff a good view of what I am capable of and the types of things I can offer the server.
Previous experience, and if yes, please provide the IPs?
This is an important section as well. You want to know if your possible Admin has been staff on any other servers before yours and if they're still on them because it could affect the player's activity and their dedication.
Any additional info?
It is always nice to know a small bit about your possible Admin's life or where they live or what their hobbies are etc.
